Case managers play a vital role in the long-term care of individuals in recovery. Our case managers will assess cases to determine an individualized action plan for each case. Action plans will vary on a case-by-case basis.
Upon arrival, a dedicated case manager conducts a thorough and compassionate assessment of all the client’s major needs. This initial evaluation involves understanding the client’s mental, emotional, and physical health, as well as any past experiences with addiction treatment, support systems, and personal goals. By taking the time to assess these critical aspects, we are not only able to tailor a personalized plan of action but also build a trusting relationship that fosters open communication. Together, the client and case manager will create a prioritized list of goals, addressing both immediate needs—such as establishing a sober routine—and longer-term objectives like securing stable employment and building healthy relationships.
To empower our clients in their recovery journey, we provide a user-friendly app that allows them to keep track of their progress and goals right from their smartphones. This innovative tool is designed to enhance engagement by providing daily reminders, motivational messages, and easy access to resources that can help with their recovery journey. Clients can input their daily activities, achievements, and any challenges they face, while our case managers can monitor their advancements in real time. This seamless tracking facilitates open discussions during weekly follow-ups, enabling immediate adjustments to their recovery plans when needed.
Our commitment to our clients doesn’t end after the initial assessment. Each case manager follows up weekly, maintaining open lines of communication through in-person meetings, phone calls, and messaging within the app. This ongoing support is crucial for bridging the gap between treatment and independent living. We believe that accountability is key to success, and case managers help individuals stay focused on their goals while gently guiding them through setbacks. This includes helping clients find a sponsor, securing employment, and actively engaging in their recovery programs, such as 12-step meetings or therapy sessions.

Mastermind Recovery (MMR) has a unique case management structure to include follow up care to assess the indelible impression we have left with our applicants. Our goal is to see individuals make it in recovery long term by a genuine transformation of their mindset. All case management evaluations will be reviewed by a double board-certified physician psychiatrist and an addiction psychiatrist.
